Save Time By Bringing In Your Forms Filled Out

When preparing for a visit to Acadiana Urgent Care Center, we ask that you bring a driver’s license (or a picture ID) and your insurance and/or Medicare cards. We also ask that you know any medications you are currently taking and any medication allergies you may have.

Save time when arriving by printing and filling out the New Patient Information Form or the Established Patient Information Form before coming to the Center.
